Online Karate Camp For Beginners

I understand that under the terms of this agreement, the school obligates itself to furnish me with competent instruction and suitable facilities for teaching lessons. Qualified personnel trained in the procedures and traditions of the martial arts supervise all class sessions. The regular hourly tuition is $60.00 per hour but a substantial discount has been provided to me by virtue of enrolling in a regular course. 

The student, or parent of the student in the case of a minor, hereby represents that the persons taking the prescribed course of instruction is/are physically fit to take this course. I understand and agree that the school will not be held liable for injuries, damages, etc., not caused by or resulting from the negligence of the owners, operators employees or persons in charge of such establishment. Be advised that the students should arrive no more than ten (10) minutes prior to the beginning of their scheduled classes, and should be picked up within ten (10) minutes of the end of classes for their own safety. I acknowledge receiving a copy of this agreement. 

The student(s) will faithfully comply with all the martial arts traditions and rules of conduct at the school. Any breach of conduct may result in a suspension of training privileges or expulsion from the school. I further understand that failure to complete my lessons does not relieve me of my obligation to pay the tuition in full.

As required by General Obligations Law, you have certain rights to cancel this contract. These are set forth in full on the reverse side of this contract and are made a part of this agreement. You may cancel this agreement without any penalty or further obligation within three (3) days from the date of this agreement. Notice of cancellation shall be in writing and mailed to the school by certified or registered mail --7222 Muncaster Mill Rd. Derwood, MD 20855. I understand my rights as stated above. Note: Tuition payments will be administered by Pandas Karate. This name will appear on your bank/credit card statement. Any account more than five (5) days past due will be charged a $10.00 late fee. All late payment and/or banking fees will be added to the buyers balance and collected from them. Any returned Bank Draft or check will be charged a $25.00 fee; credit card of debit card chargebacks will be assessed at $35.00. Pandas Karate. are not responsible for any bank fees incurred by the buyer. Pandas Karate. has the right to modify any payment due date and resubmit returned/declined items without prior notice to buyer. 

If the Pandas Karate is closed for a month or more, you are entitled to your choice of either an extension of the agreement or a prorated refund. If the closing is not the fault of the business, we are entitled to choose either the extension or refund. 

1. You have the right to cancel this agreement within three business days after receipt of a copy of this agreement. Cancellation must be in writing, and delivered in person or by certified or registered mail. If you cancel, you are entitled to a full refund of all monies paid. 



2. If you become disabled for at least 3 months during the membership term and the disability is confirmed in writing by a physician, you are entitled to an extension of the agreement. Since we are exempt from the bonding requirement, we cannot collect payments during a member’s disability extension so that we are not holding more than three months of payment in advance. 

This notice is an integral part of the application and contract for membership.
